OFFICE POLICIES

To ensure a respectful, efficient, and high-quality experience for all patients, we ask that you review and adhere to the following policies:

  • Timely Arrival
    We value your time and the time of others. Please arrive promptly for your appointment to preserve the integrity of our schedule.

  • Late Arrivals & Extended Visits
    Patients arriving more than 10 minutes late may be rescheduled at the provider’s discretion.
    Visits extending beyond the scheduled time may incur a $25 fee. This fee is not billable to insurance and is the patient’s responsibility.

  • Individualized Appointments
    Each appointment is reserved for the scheduled patient. If another family member requires care, we are pleased to arrange a separate visit.

  • Missed Appointments
    Two consecutive missed appointments without notice will result in temporary suspension of services. Notification will be provided in writing.

  • Membership Status
    Membership must remain current to maintain active patient status. Accounts more than two months past due may be suspended, with written notice and a 30-day opportunity to resolve the balance before termination.

  • Prescription Refills
    Please submit refill requests at least one week in advance through the Patient Portal or Klara. All requests must be made in writing.

  • Wait Times
    If you have been waiting more than 15 minutes past your scheduled time, please notify our team so we may assist you promptly.

  • Documentation Requests
    School or workplace absence documentation is provided at the provider’s discretion.

  • Patient Portal
    Active use of our Patient Health Records (PHR) portal is required for all patients to ensure secure and efficient communication.

  • Supplements
    We offer select provider-recommended supplements for your convenience. While you are under no obligation to purchase through our office, we cannot guarantee products obtained elsewhere.

  • Superbills
    For out-of-network insurance plans, superbills may be provided at the discretion of our billing department. We do not routinely issue superbills for self-pay patients.

  • Returns
    Unopened products may be returned within 30 days of purchase with proof of receipt.

The below fee schedule is applicable to all new appointments effective immediately. This is a general list of our most common charges and may not include fees associated with a visit due to extenuating circumstances. Pricing is determined by the provider at the time of service and is based on the time and complexity of each visit.

30 Minute New Pediatric Patient (Establish Care)*

Includes a 30-minute comprehensive office visit with a provider. *The Establish care visit only applies to patients with no immediate health concerns.

 

30 Minute Follow Up

 

$100.00 $145.00

Includes a 30-minute comprehensive office visit to adjust or evaluate an ongoing treatment plan.

Sick Visit

 

$100.00 $145.00

Includes a 15-30 minute focused visit to address an immediate illness.

 

Well Baby Visits

 

$89.00 - $94 $190-$215

These visits are conducted in a group “centering” style appointment.

 

Annual Wellness Exam

 

$150.00 $190-$216

Includes sports or work physicals, these are routine wellness exams.

Telemedicine Appointment

 

Telemedicine appointments are billed at a variety of rates depending on the complexity of your clinical case and the length of the appointment. Any appointment that exceeds the 30-minute appointment or reflects a higher complexity level will be billed accordingly. 

Release of Medical Records

Requests for medical records released to individuals will be billed at $10-25 depending on the complexity and volume of the records. You will be notified of the charge prior to running it.

Some in-office procedures will incur additional charges but will be discussed prior to starting.

IF YOU NEED TO CANCEL A SCHEDULED APPOINTMENT, YOU MUST DO SO 24 HOURS PRIOR.

Text, email, or call to let us know that you no longer require your scheduled appointment so that someone else can be seen.

If you miss a scheduled appointment without notice, you will be charged for the visit.


Membership Cancellation Policy:

There is no contract or minimum enrollment period. However, if a member cancels his/her membership there is a 9-month waiting period to reenter the practice. If for some reason the member needs to re-enroll sooner, there is a $100 readmission fee per patient.

If you wish to cancel your membership, you must email notice that you wish to do so prior to the 5th of the month. Cancellation requests after the 5th of the month will be charged half of the full membership and all emails received after the 18th will be charged for the entire month.

If you cancel your membership or your membership is canceled with a balance due, we reserve the right to refer your past due balances to collections.

Cancellations MUST be done in writing to membership@sccimed.org.

Emailed cancellation requests need to include the names and birth dates of the membership being canceled and a brief explanation of the termination.
